What the job really is

As a Pediatric Home Care Specialist (Paramedic Certified) at Imagine Pediatrics, you will provide in-home medical, behavioral, and social care to pediatric patients in collaboration with an interdisciplinary team. Your role involves traveling to members' homes to address acute needs, conducting clinical intake visits, and working to prevent hospital readmissions by engaging with recently discharged members. You will utilize an integrated technology platform to enhance care delivery alongside other healthcare professionals.
