Hybrid Nurse Practitioner

As a Nurse Practitioner at GuideStar Eldercare, you will provide care to patients with dementia and neurocognitive disorders in nursing homes and assisted living facilities. Your role involves conducting evaluations, therapy services, and cognitive testing, while collaborating with a multidisciplinary team and maintaining relationships with patients and their families. You will have the autonomy of private practice with support from corporate leadership and administrative teams.
