Nurse Practitioner

As a Nurse Practitioner at GuideStar Eldercare, you will provide care for patients with dementia and other neurocognitive disorders in nursing homes and assisted living facilities. Your role involves conducting evaluations, providing therapy services, and maintaining documentation, all while collaborating with a multidisciplinary team. You will have the autonomy of private practice supported by corporate clinical leadership and administrative teams, and you will travel daily to assigned facilities from your home.
