Nurse Practitioner

As a Nurse Practitioner at GuideStar Eldercare, you will provide care for patients with dementia and other neurocognitive disorders in nursing homes and assisted living facilities. Your role involves conducting evaluations, therapy services, and cognitive testing, while maintaining relationships with patients and their families. You will work autonomously with support from a regional Area Manager and collaborate with a multidisciplinary team to promote patient safety and dignity.
