Care Manager

The Care Manager will work closely with primary care practices to provide telephone-based health coaching, quality improvement, and care coordination for Medicare patients. This role involves collaborating with an interdisciplinary care team and utilizing Aledade's proprietary tools to manage high-risk patients and improve their health outcomes through better understanding of chronic conditions and access to appropriate care.
