Home Care Coordinator - Cantonese/Mandarin (RN License)

The Home Care Coordinator for PACE will assess and coordinate home care services for elderly participants, ensuring they receive the necessary support to age safely in their communities. This role involves conducting home care assessments, supervising interventions, and providing education on home safety to participants and caregivers as part of an interdisciplinary team.
