The PACE Clinic Assistant at Asian Health Services provides nursing care to elderly participants, executing interventions under supervision and evaluating medical conditions both in-person and via phone. This role involves educating participants and caregivers, coordinating care transitions, scheduling appointments, and supporting quality improvement initiatives within the clinic.
