Outreach and Enrollment Coordinator (Cantonese/Mandarin)

The Outreach and Enrollment Coordinator serves as the initial contact for potential participants in the PACE program, guiding them through the eligibility screening, intake, and enrollment processes. This role involves explaining PACE, Medicare, and Medi-Cal services, gathering necessary documentation, and scheduling assessments and tours to ensure a smooth enrollment experience for seniors and their caregivers.
