Clinical Care Manager (Registered Nurse)

The Clinical Care Manager (Registered Nurse) at Asian Health Services will oversee the daily operations of the PACE clinic and manage a panel of elderly participants. Responsibilities include conducting nursing assessments, developing care plans, coordinating care across various settings, and managing chronic conditions to enhance participants' safety and quality of life.
