Intake Coordinator - Cantonese/Mandarin (RN License)

The PACE Intake Coordinator at Asian Health Services is responsible for facilitating access to the PACE model of care for older adults and their caregivers. This role involves setting intake appointments, documenting medical needs, and gathering essential information to ensure a smooth transition into coordinated care, working independently both over the phone and in-person at participants' homes or the PACE Day Center.
