Mental Health Care Coordinator for High Needs Youth

The Mental Health Care Coordinator, also known as the Intensive Care Coordinator (ICC), is responsible for assessing and coordinating care plans for youth with complex behavioral health needs. This role involves collaborating with families and community partners to form Child & Family Teams, managing crises, and providing behavioral health services in various settings. The ICC will also facilitate meetings, document progress, and ensure that interventions are tailored to each client's unique needs.
