  • Master's or doctorate Degree
  • Active state physician license without restrictions
  • State CDS certification and DEA registration
  • Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter
  • Must be eligible for enrollment to provide services to Medicare and Medi-Cal participants

What the job really is

As a Primary Care Physician and Pod Leader, you will provide community-based care to vulnerable seniors, focusing on building long-term relationships with patients. Your role will involve conducting comprehensive assessments, coordinating care plans with an interdisciplinary team, and managing an average of 8 patients per day without hospital rounds. You will also have the opportunity to work flexible hours, including home visits, while contributing to a value-based care model.
