Care Coordinator( 8am-5pm EST)

As a Care Coordinator at TailorCare, you will be the first point of contact for patients, handling inbound calls and facilitating communication between care teams. Your responsibilities will include answering patient inquiries, resolving issues, collecting documentation, and ensuring smooth care coordination through accurate documentation and timely follow-ups.
