Clinical Care Navigator, Outbound (Saturday-Wednesday 9am-5pm ET)

As a Clinical Care Navigator at Spring Health, you will provide clinical support and psychoeducation to members through outbound calls, assisting them with mental health, substance use, and relationship concerns. Your role includes assessing risks, providing crisis support, coordinating care, and documenting interactions, all while collaborating with multi-disciplinary teams to ensure a smooth member experience. You will also participate in staff meetings and help onboard new team members.
