What the job really is

The Case Manager is responsible for managing the entire case management process for consumers with developmental disabilities, including assessment, planning, advocacy, and monitoring services. They develop and implement Individual Program Plans (IPP) or Individual Family Service Plans (IFSP) and ensure that consumers receive the necessary services and support. The role involves coordinating with families and practitioners, maintaining documentation, and advocating for consumer rights.
