Primary Care Nurse Practitioner

The Primary Care Nurse Practitioner/Physician’s Assistant will provide on-site primary care services to residents in long-term care and skilled nursing facilities. Responsibilities include performing physical exams, diagnosing and treating health conditions, developing care plans, prescribing medications, and collaborating with facility staff to ensure high-quality care for seniors.
