Social Worker - Adult Mental Health Team

The Social Worker for the Adult Mental Health Team in Portsmouth will support adults facing mental health challenges by promoting their independence and wellbeing through person-centred social work. Daily tasks include conducting assessments, developing support plans, managing a varied caseload, and collaborating with health professionals and community services to ensure individuals receive necessary support.
