Contracted In-Home Occupational Therapist

As a Contracted In-Home Occupational Therapist at Jukebox Health, you will conduct in-home assessments and functional evaluations for older adults and individuals with disabilities. Your role involves delivering personalized recommendations for home modifications, educating clients and caregivers on safety techniques, and collaborating with a remote team to improve processes. You will also be responsible for maintaining accurate documentation through an electronic system.

• Compensation of $120 per evaluation
• Contractors are paid on a monthly basis, within 2 weeks of the previous month’s end
• Travel to/from client’s home is compensated
• Evaluation and documentation time is compensated
• Additional pay for extended travel, with prior discussion and approval

• Anticipated start date is not until 10/1/26
• Contracted position with a minimum of 10 hours of weekly availability
• Compensation is per completed evaluation, which may vary based on workload
