Contracted In-Home Occupational Therapist

As a Contracted In-Home Occupational Therapist at Jukebox Health, you will conduct in-home assessments and functional evaluations for older adults and individuals with disabilities. Your role involves delivering personalized recommendations for home modifications, educating clients and caregivers on safety techniques, and collaborating with a remote team to improve processes. The position requires a minimum of 10 hours of availability per week and involves travel to clients' homes for evaluations.

• Compensation of $120 per evaluation
• Contractors paid on a monthly basis, within 2 weeks of the previous month’s end
• Payment includes travel to/from client’s home, the evaluation itself, and documentation
• Additional pay for extended travel if needed

• Anticipated start date is not until 10/1/26
• Position is contracted with a minimum of 10 hours weekly availability
• Compensation is per completed evaluation, which may affect income stability
