The Home Health Intake/Scheduling Coordinator will be responsible for maintaining and coordinating schedules for home health services, processing incoming referrals, and ensuring timely case assignments. The role requires effective communication with clinicians, patients, and referral sources, as well as managing high call volumes and responding to urgent scheduling needs in a fast-paced environment.
