Nurse Practitioner (Remote, IL License Required)

As a Nurse Practitioner at Galileo, you will provide high-quality, comprehensive follow-up care to patients recently discharged from the hospital. Your day-to-day responsibilities include conducting telephonic outreach and scheduled video visits, focusing on medication reconciliation and symptom screening. You will also coordinate care across various settings and collaborate with nursing staff to meet individual and team health goals.
