Nurse Practitioner (Remote, SC License Required)

As a Nurse Practitioner at Galileo, you will provide high-quality, comprehensive follow-up care to patients post-hospital discharge through telephonic outreach and scheduled video visits. Your responsibilities will include medication reconciliation, symptom screening, and coordinating care across various settings while building trust with patients. You will work collaboratively with a nursing team and be accountable for individual performance targets within a population health framework.
