Remote Pharmacy Care Coordinator, MA (Contract to Perm)

As a Telephonic Pharmacy Care Coordinator at Evergreen Nephrology, you will engage with patients over the phone to introduce and facilitate enrollment in a specialized pharmacy service. Your role involves high-volume outbound outreach, obtaining patient consent, documenting interactions in the electronic health record, and ensuring patients receive their medications on time. You will work remotely, focusing on building trust and effectively communicating the value of the pharmacy services to patients living with chronic kidney disease and other complex conditions.
