Remote Care Coordinator (Pacific Time)

As a Remote Care Coordinator at Evergreen Nephrology, you will support proactive care coordination for patients with chronic conditions, particularly chronic kidney disease. Your daily tasks will include conducting patient outreach, maintaining care plans, ensuring documentation compliance, and facilitating communication between patients and healthcare providers. This role requires strong organizational and communication skills, as well as the ability to manage a high-volume patient panel.
