Remote Care Coordinator, LPN

As a Remote Care Coordinator, LPN at Evergreen Nephrology, you will conduct monthly clinical check-ins with patients, assess their clinical conditions, and reinforce care plans. You will collaborate closely with healthcare providers, manage patient documentation, and ensure compliance with care protocols while engaging patients primarily through SMS and phone outreach. This role requires strong clinical judgment and the ability to work effectively in a remote environment.
