Program Manager, Chronic and Complex Care

As the Program Manager for Chronic and Complex Patient Management at Evergreen Nephrology, you will lead a team dedicated to providing patient-centered care for individuals with chronic conditions, particularly kidney disease. Your responsibilities will include overseeing daily operations, ensuring adherence to care pathways, improving program performance, and supporting team development through coaching and training. You will also prepare performance reports and foster relationships with key stakeholders to enhance patient outcomes.
