Remote Care Coordinator (Mountain Time)

As a Remote Care Coordinator at Evergreen Nephrology, you will engage with patients living with kidney disease to ensure they receive comprehensive and person-centered care. Your role involves coordinating care plans, addressing social determinants of health, and advocating for patients while collaborating with an interdisciplinary team. You will also perform outreach, facilitate resource navigation, and maintain communication with patients and healthcare teams to enhance health outcomes.
