Service Coordinator - Family Networks

The Service Coordinator will support children and families by coordinating services and facilitating placements, ensuring smooth transitions from residential to home-based and community care. This role involves assessing DCF referrals, overseeing emergency placements, and linking service plans with treatment plans while promoting quality improvement and compliance with policies. The ideal candidate will work collaboratively with families, providers, and state partners in a trauma-informed manner.
